Decision record (ADR)

A short document of one significant design choice with its context, the options considered, the decision, its consequences and the conditions for reconsidering it.

In detail

The format popularised as the architecture decision record keeps one decision per record with a status such as proposed, accepted or superseded. A superseded record is kept and linked to its replacement, so the history shows why the design changed.
